Prairie Community Services, Inc. (PCS) provides Adult Foster Care (AFC) services throughout the state of Minnesota. Our services are provided in a community residential setting to individuals ages 18 years of age or older. We serve individuals with intellectual disabilities, mental illnesses, traumatic brain injuries, and other related conditions. Each of our residential programs serves 2-4 individuals. Staffing at each program is based on needs of the individuals served. PCS has one Intermediate Care Facility for individuals with intellectual disabilities (ICF/IID) home located in Fergus Falls, MN. This home is licensed under the Department of Health and serves up to 5 individuals. Individuals at this home must be 18 years of age or older and must qualify under Department of Health licensing standards of being diagnosed with an Intellectual Disability prior to the age of 22.
